On September 2, 2015, a data breach occurred at ClearVoice Surveys, a survey and data collection company. The breach exposed 15,074,786 records, including personally identifiable information (PII) such as names, addresses, email addresses, and phone numbers. The breach also included sensitive financial information, such as credit card numbers and Social Security numbers.

The breach was caused by a vulnerability in ClearVoices software that allowed attackers to access the companys database and steal the data. ClearVoice did not announce the breach until September 15, 2015, more than two weeks after it was discovered. This delay allowed attackers to exploit the data for malicious purposes, such as identity theft and financial fraud.
